• Coordinate and facilitate engagement initiatives with internal and external stakeholders to strengthen care integration across hospitals, primary care, and community services.
• Provide comprehensive secretariat and administrative support, including stakeholder coordination, project management, preparation of meeting minutes, reports, and presentations, as well as tracking follow-up actions.
• Support the formulation of short‑ and long‑term clinical and population health strategies by mapping existing services, identifying care gaps, and analysing relevant data to inform service planning, prioritisation, and KPI development.
• Assist in project and grant management for assigned population health programmes to ensure effective execution.
• Collaborate closely with programme teams, Legal, and Finance to facilitate work plan development, budget preparation, contract execution, and timely claims submission.
• Partner with internal and external stakeholders to consolidate information and prepare regular management and committee reports.
• Support audit and review processes, contribute to programme evaluations, and assist in developing strategies to ensure long‑term sustainability and impact.

• Degree in any relevant field.
• Min 3 years of working experience prior to the healthcare industry.
