LPN- Administrator

Human Touch Home Care seeking an Administrator to function as an exceptional Admin manager to oversee care management, quality assurance and compliance for the home care services, and direct the interdisciplinary team (IDT). An LPN to review and update care plans and documentation while maintaining accurate records. The Administrator /LPN will establish goals and objectives to advance the home care agency to higher levels of quality and regulatory compliance while implementing quality standards to ensure excellent delivery of home health care.

• Lead, direct and support a team of Caregivers
• Work as a team to uphold the company's mission, vision and core values
• Develop, maintain, and implement nursing policies and procedures
• Ensure compliance with local, state, and federal regulations; as well as company policies & procedures
• Promote and champion employee and patient safety/ well being
• Monitor/audit staff practices and implementation for quality assurance
• Evaluate the company's caregiver program and individual performance to ensure the best possible patient clinical outcomes and satisfaction

• A minimum of 2 years of Administrator level experience in home healthcare
• Minimum of 2 years of experience as a supervisor/manager
• Bachelor's degree in nursing or health related field preferred
• Licensed Practical Nurse (Preferred)
• Prior experience with computer/software driven quality programs is a plus
• Understanding of and demonstrated success in achieving and maintaining Medicare/Medicaid standards
• Excellent organization skills with the ability to multi-task effectively

$50,000-$60,000/YR
