In-Home Health - Nurse Practitioner or Physician Assistant Fort Collins

Job Overview

Signify Health, part of CVS Health, is looking for a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations for members. The role involves visiting individuals in their homes and offering comprehensive health assessments.

The evaluation visit lasts approximately 45 minutes and includes a medication and medical history review, a physical examination, and any additional diagnostic tests ordered by the member’s health plan. Clinicians use an iPad and a clinician‑friendly workflow for each assessment and are not required to prescribe medicine, order lab tests, or alter members’ current treatment regimens.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).

As an in‑home clinician, your primary workplace is the private residence of members. You must be able to safely perform duties in various home settings that may include pets, secondhand smoke, or other potential allergens or irritants.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Able to cover the following primary locations:
  Fort Collins.

Pay range: $87,035.00 – $ (base hourly rate or annual salary depends on experience, education, geography, and other factors).
