Compassionate, kind, and empathetic to your patients and their families. - Nurse/Patient Relationship:
Develops and maintains a therapeutic relationship through the health care continuum. - Provides direct patient care services to hospice patients.
- Empowers patient's caregivers by educating them on care for their loved one.
- Provides coordination of care, ensuring continuous assessment of each patient's and family's needs.
- Responsible for documentation and communication of care, medication, and observations made during care of the patient to track responsiveness and health.
- Responsible for providing appropriate patient/family education to help them achieve a pain‑free and comfortable status.

Monday through Friday 8:00am – 5:00pm. On‑call rotation with additional pay.
Pay$92,000 – $98,000 per year depending on experience.
Qualifications- Registered nurse with current licensure to practice professional nursing in the state.
- Graduate of a National League for Nursing accredited school of nursing.
- Maintains a current CPR certification.
- Current valid driver’s license.
- At least one year of hospice experience (preferred).
- Demonstrates excellent observation, problem‑solving, verbal and written communication skills, and nursing skills per competency checklist.
- Shows ability to organize and prioritize workload independently.
