What you will do
- Carry out assessments of adults’ care and support needs, ensuring robust and effective support is in place.
- Develop, review, and monitor care and support plans, adapting as necessary to meet individual needs.
- Collaborate with adults, their families, carers, and other professionals to identify suitable outcomes and promote independence.
- Undertake risk assessments and respond decisively to safeguarding concerns, ensuring welfare is paramount.
- Apply relevant adult social care legislation and statutory guidance, maintaining compliance and safeguarding standards.
- Engage in collaborative work with health and social care professionals and partner agencies to achieve consistent and positive results.
- Maintain comprehensive case records and documentation, ensuring accuracy and timeliness.
- Manage an allocated caseload effectively, prioritising tasks by need and risk.
- Participate actively in reviews and ensure that agreed actions are executed efficiently.
- Contribute to effective multi-agency working, driving positive outcomes for adults in your care.
About You
- Qualified Social Worker with experience supporting adults.
- Possess a Social Work qualification and current Social Work England registration.
- Relevant experience and knowledge of adult social care legislation and safeguarding.
- Demonstrated ability to manage a caseload effectively.
- Strong communication skills and a collaborative attitude.
- £33.00 per hour
- 37 hours per week
- Start: 24 August 2026
- Location:
Thornton Library, FY5 - Interview required, with CV review forming part of the selection process.
