Clinical Supervisor RN – Hospice Care in Thousand Oaks Join a Compassionate Team Making a Difference Every Day

We are seeking an experienced and compassionate Registered Nurse Clinical Supervisor to lead and support our hospice care team in Thousand Oaks. In this leadership role, you will oversee the coordination and delivery of high-quality patient care while mentoring clinical staff and ensuring exceptional service for patients and families during life’s most meaningful moments.

The annual salary range for this position at commencement of employment is expected to be between $110,000 - $125,000; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.

Position Summary

The Clinical Supervisor is responsible for the planning, coordination, supervision, and delivery of hospice and palliative care services. This role provides daily clinical leadership and operational support to interdisciplinary team members caring for patients with terminal illnesses.

The Clinical Supervisor works closely with physicians, nurses, caregivers, and families to ensure quality care, regulatory compliance, and compassionate support throughout the patient journey.

This position must be available on-site or accessible by telecommunications during operating hours.

Clinical Leadership & Patient Care
  • Supervise and coordinate daily patient care activities and staff assignments
  • Provide clinical guidance and support to hospice field staff
  • Visit patients as needed to assist with complex cases, staff supervision, and specialized care
  • Ensure physician-ordered services are delivered appropriately and efficiently
  • Monitor utilization of hospice levels of care including routine, respite, continuous care, and GIP
  • Participate in the development and review of individualized patient care plans
  • Conduct concurrent chart reviews for quality, compliance, and accuracy
  • Monitor timely recertifications, discharges, and visit frequencies
Staff Supervision & Development
  • Orient, mentor, and support new clinical staff
  • Conduct ongoing performance evaluations and supervisory visits
  • Coordinate staff education and in-service training programs
  • Serve as a clinical resource and promote professional growth among team members
  • Lead or participate in interdisciplinary group (IDG) meetings
Quality & Compliance
  • Ensure compliance with hospice regulations, standards, and agency policies
  • Support quality improvement and performance improvement initiatives
  • Assist in developing and implementing agency policies and procedures
  • Maintain high standards for documentation, customer service, and patient satisfaction
Additional Responsibilities
  • Participate in administrative on-call rotation as assigned
  • Coordinate supplies, equipment, and community resources for patient care
  • Maintain professional communication and collaboration with patients, families, physicians, and team members
  • Support the mission, vision, and values of the organization
  • Perform additional duties as assigned
Qualifications
  • Current and active California Registered Nurse (RN) license
  • Minimum of 2 years of experience within the last 5 years in hospice, home health, primary care, or healthcare facility settings
  • Strong clinical decision‑making and leadership skills
  • Knowledge of hospice regulations and standards including HCFA, Title 22, and JCAHO requirements
  • Current CPR certification
  • Valid driver’s license, reliable transportation, and proof of auto insurance
  • Ability to communicate effectively in English, both verbally and in writing
Preferred
  • Previous hospice leadership or supervisory experience
  • Experience leading interdisciplinary teams
  • Strong organizational, documentation, and problem‑solving skills
Physical Requirements
  • Ability to perform assisted patient transfers with or without transfer devices
  • No lifting or moving restrictions
  • Ability to walk, stand, bend, and travel to patient homes or facilities as needed
