Position Summary

The Registered Nurse (RN) provides skilled nursing services to patients in their homes, helping them achieve the highest level of health, independence, and quality of life. This role involves patient assessments, care planning, coordination with physicians and interdisciplinary team members, patient education, and case management responsibilities.

Our nurses are empowered to use their clinical expertise while building strong relationships with patients and families throughout their recovery journey.

Key Responsibilities:

Patient Care & Clinical Excellence
  • Perform comprehensive initial and ongoing patient assessments.
  • Develop, implement, and update individualized plans of care.
  • Monitor patient progress and modify care plans as needed.
  • Administer medications, treatments, and skilled nursing interventions as ordered by the physician.
  • Provide disease management education and health coaching to patients and caregivers.
  • Identify discharge planning needs and facilitate smooth transitions of care.
  • Serve as Case Manager when assigned, coordinating care for an established patient caseload.
Care Coordination & Communication
  • Collaborate with physicians, therapists, home health aides, and other healthcare professionals.
  • Communicate changes in patient condition and obtain physician orders when necessary.
  • Maintain accurate and timely clinical documentation.
  • Participate in interdisciplinary care planning and patient advocacy.
Leadership & Support
  • Supervise and evaluate Home Health Aide services in accordance with regulatory requirements.
  • Coordinate equipment, supplies, and community resources necessary for patient care.
  • Participate in on-call rotation as needed.
Qualifications Required
  • Current California Registered Nurse (RN) license.
  • Graduate of an accredited nursing program.
  • Current CPR certification.
  • Valid California driver's license and reliable transportation.
  • Minimum one (1) year of recent nursing experience.
  • Excellent clinical assessment, communication, and documentation skills.
Preferred
  • Home health experience.
  • Bachelor's Degree in Nursing (BSN).
  • Case management experience.
  • Familiarity with electronic medical records (EMR).

Physical Requirements
  • Ability to travel to patient homes.
  • Ability to lift, transfer, and assist patients safely.
  • Frequent standing, walking, bending, reaching, kneeling, and lifting of equipment and supplies.
  • Visual and hearing acuity sufficient to perform nursing responsibilities.
