We partner with 380+ assisted living, independent living, memory care, and skilled nursing communities nationwide, delivering relationship-based, resident-centered care that prioritizes dignity, access, and outcomes for aging populations.
- Conducting comprehensive psychiatric evaluations for geriatric patients in home and facility-based settings.
- Diagnosing and managing mood disorders, anxiety disorders, dementia-related behavioral symptoms, and other psychiatric conditions.
- Prescribing and managing psychotropic medications with careful consideration of comorbidities and geriatric physiology.
- Providing crisis intervention, behavioral guidance, and family education when clinically indicated.
- Collaborating with primary care providers, therapists, and facility care teams to ensure coordinated, whole-person care.
- Building trusted relationships with patients, caregivers, families, and community partners.
- Maintaining consistent, accurate clinical documentation and follow-through.
- Guaranteed Base Salary + Productivity
- Total annual compensation range: $150,000–$200,000+, based on experience, caseload, and availability.

- NP/PA:
Current, active, and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in good standing in the state of practice required and must be maintained throughout employment. - Experience in geriatric psychiatry, long-term care, or mobile healthcare preferred.
- Strong clinical judgment, communication skills, and comfort working independently with remote support.
