• Home health setting serving a geriatric patient population
• Day shift schedule, 5x8‑hour shifts (8:00–17:00)
• Average caseload of 5–7 patients per day, to be confirmed during interview
• Productivity expectation of 6 points per day / 30 points per week
• Territory covers up to a 60‑mile radius from the hub/office; flexibility with territory is expected
• EMR: HCHB (Homecare Homebase) experience required
• iPad provided for clinical use
• Mileage reimbursed at the IRS standard rate
• Local candidates accepted at the travel rate
• Orientation held on Tuesdays; modules completed during orientation
• Timely documentation and communication are mandatory
• On‑call rotation may be required – discuss during interview
• No scheduled on‑call requirement as part of the standard assignment
• Travel pairs accepted; each traveler assigned own territory and requires own transportation
• Former permanent employees must have been separated for at least 6 months of HR approval before returning as a traveler
• Requested time off (RTO) must be submitted at application time
• Scrubs required in any color; jeans not permitted
• Personal cell phone not required; an iPad will be provided

• Active OT license required; license in hand required for consideration
• Minimum 1 year of recent, relevant OT experience required
• OASIS experience required for staff completing Start of Care (SOC)
• Proficiency in HCHB (Homecare Homebase) required
• Current BLS certification required
• Active driver's license and personal vehicle required
• First‑time travelers will not be considered for this position
