Travel Physical Therapist - Home Health

American Traveler is seeking a Physical Therapist with home health experience, OASIS proficiency, and HCHB EMR experience for a 13-week home health travel assignment.

Job Details

• Home health setting serving a geriatric patient population,

• Average caseload of 5-7 patients per day, confirmed during interview,

• Productivity expectation of 6 points per day / 30 points per week,

• Day shift schedule, Monday-Friday, 8:00 AM-5:00 PM (5x8-hour shifts),

• Territory covers up to a 60-mile radius from the hub/office; flexibility with territory is expected,

• No on-call requirement, though an on-call rotation may apply - to be discussed during interview,

• HCHB (Homecare Homebase) used for documentation,

• iPad provided for use in the field,

Job Requirements

• Minimum 1 year of recent, relevant physical therapy experience in home health,

• OASIS experience required for candidates completing Start of Care (SOC),

• Proficiency in HCHB (Homecare Homebase) required,

• Current BLS certification required,

• Active driver's license and personal vehicle required,

• Must have PT license in hand at time of consideration - license required if applicable,

• First-time travelers will not be considered for this position,

Additional Information

• Orientation is held on Tuesdays and includes completion of required modules,

• Timely documentation and communication are mandatory throughout the assignment,

• Scrubs required in any color; jeans are not permitted,

• Mileage is reimbursed at the IRS standard rate,

• Local candidates are accepted and are eligible for the travel rate,

• Travel pairs are accepted; however, each traveler will be assigned their own territory and must have independent transportation,

• Former permanent employees must have been separated for a minimum of 6 months before returning as a traveler, and are subject to HR approval,

• Any requested time off must be submitted at the time of application,
