We are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Recertification Nurse (RN) to join our agency and serve existing and returning clients throughout North West Ohio
, with primary service area around Tiffin, OH and coverage up to a 2-hour radius surrounding communities.
- Coordinate, reevaluate, and confirm the medical necessity of continued home health care services for patients.
- Provide patient care and ensure optimal outcomes; oversee patient services and nursing care with core skills in managed care, patient assessments, compliance, care planning, and discharge planning.
- Submit updated documentation of the patient s condition and ongoing care needs in a timely manner and in compliance with state, federal, and agency guidelines.
- Facilitate communication among physicians, agency staff, service administrators, other caregivers and clinicians, and the client and family.
- Travel and scheduling: discuss exact locations and daily shifts based on availability; typical hours are Monday–Friday, 8am–5pm; mileage reimbursement is provided.
Regional Program Administrator, or Clinical Administrator/Director of Nursing.
Qualifications- Graduate of an accredited School of Nursing and active RN license to practice in Ohio.
- Minimum 1 year of RN experience; 1 year in home health or equivalent setting.
- CPR and First Aid certifications (or obtainable prior to hire).
- Experience with OASIS and NON-OASIS assessments; strong nursing, patient care, and confidentiality background.
- Strong attendance and dependability; reliable, professional, compassionate, detail-oriented, accountable, friendly, respectful, and positive.
- Familiarity with HIPAA regulations and medical terminology; current driver s license and reliable transportation; willingness to travel to clients in multiple counties.
- Local travel: 30–60 minute daily radius; occasional 1–2 hour longer trips; overtime may be available but not required.

- Hourly Wage: $35–$40/hour

Negotiable based on travel radius and average required recertifications per week
As an RN Recertification Nurse, you will coordinate, re-evaluate, and confirm the medical necessity of continued home health care services, provide direct patient care, and ensure optimal outcomes.

Reports to: Regional Program Administrator or Clinical Administrator/Director of Nursing
Required Experience & Licensure- Graduate of an accredited School of Nursing; active RN license in Ohio.
- Minimum 1 year RN experience; 1 year in a home health setting preferred.
- CPR and First Aid certifications; experience with OASIS and NON-OASIS assessments required.
- Wound care, labs, and assessment experience a plus; HIPAA familiarity and strong confidentiality standards.
- Reliable transportation and willingness to travel across counties.
