RN Recertification Nurse - Home Health Agency Tiffin, OH - NW OH

Overview

Home Care Network, Inc. (HCN) is a nurse-owned and locally led home health agency, focused on patient care and wellbeing. We seek a highly skilled, experienced RN Recertification Nurse to join our agency and serve existing and returning clients throughout North West Ohio.

• Coordinate, re-evaluate, and confirm the medical necessity of continued home health care services for our patients; provide patient care and ensure optimal outcomes.
• Manage core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning; oversee patient services and provide expert nursing care.
• Submit updated documentation of the patient s condition and ongoing care needs in a timely manner and in compliance with all state, federal, and agency guidelines.
• Ensure effective communication with physicians, agency staff, service administrators, other caregivers and clinicians, and the client and family.
• Report to:
  Regional Program Administrator or Clinical Administrator/Director of Nursing.

• Graduate of an accredited School of Nursing.
• Minimum of 1 year experience as a Registered Nurse with an active RN license to practice in the State of Ohio; 1 year of experience in a home health agency or equivalent setting preferred.
• CPR and First Aid certifications required (or obtainable prior to hire date through the agency or accredited third parties).
• Experience with OASIS and NON-OASIS assessments required.
• Strong background in nursing, patient care, and confidentiality; reliable, professional, and detail-oriented. Familiarity with HIPAA regulations and medical terminology.
• Current Driver s License and reliable transportation; willingness to travel to clients in multiple counties.

• Hourly Wage: $35 - $40/hour
• Annual Salary (+ PTO): negotiable based on travel radius and agreed-upon average recertifications per week
• Flexible, customizable schedules; no on-call requirements and no overnight shifts required.
• Autonomy in a positive, client-centered culture with Quality and Compliance support and training for all caregivers and clinicians.
• Full-time staff are eligible for quarterly bonuses, referral bonuses, PTO, health coverage, and other benefits; part-time options also available.
• Round-trip / door-to-door mileage reimbursement provided.

• Travel typically within a 30-60 minute daily radius; longer travel up to 1-2 hours may occur occasionally.
• Typical hours are Monday through Friday, 8am–5pm; travel distances and daily shifts are customizable based on availability.
• Overtime may be available but is not required.
