Role Summary
The Care Manager serves as the primary point of contact for participants and their families, coordinating Plans of Care and Service Plans that address medical, behavioral, and social needs. This role ensures that services are delivered effectively, participants remain supported in the least restrictive environment, and all care meets regulatory and organizational standards.
Key Responsibilities
- Develop and implement individualized Plans of Care and Service Plans
- Collaborate with participants, families, and providers to ensure needs are met
- Conduct monthly telephonic visits (or more frequently based on need) to monitor progress
- Update and maintain accurate documentation of care plans, visits, and service delivery
- Support transitions of care, including discharge planning and interdisciplinary meetings
- Educate participants and families about available resources and services
- Ensure compliance with all regulations, policies, and quality standards
- Participate in audits, reviews, and quality improvement initiatives
- A bachelor's degree or a Registered Nurse or Licensed Practical Nurse, properly licensed in the state of Georgia
- At least two years of relevant experience in the provision of community-based case management services
- Experience creating service plans and making recommendations for treatment or services
- Strong skills in relationship building, problem solving, and person-centered planning
- Proficiency with Microsoft Office and electronic documentation systems
- Valid driver's license, automobile insurance, and reliable transportation for frequent travel
