Your Health Organization in Georgia is seeking a Registered Nurse to join our Nurse Case Manager team, covering the Macon area. Travel to patients’ homes and facilities is required to evaluate care and initiate telemedicine visits.
Full-time, 8-hour shifts, Monday–Friday, 8am–5pm, with opportunities for telehealth as appropriate. The nurse will work within a multidisciplinary team to coordinate care, manage documentation, and ensure compliance with laws and policies.
