Health Connect America, Inc. seeks an Independent Contractor - Family Support Services to provide in home and community based support to children, adolescents, and families. Responsibilities include implementing individualized family support plans, providing skills coaching, parenting support, and resource linkage in collaboration with clinical teams. Contractors document sessions, coordinate with case managers and community partners, and maintain professional boundaries and confidentiality.

This role suits mission driven professionals who value client centered care, flexible schedules, and multidisciplinary teamwork in behavioral health and social services.

Responsibilities

  • Provide in-home and community-based family support services to children, adolescents, and families.
  • Implement and follow individualized service or family support plans in coordination with clinical staff.
  • Offer parenting support, skills coaching, and linkage to community resources and services.
  • Collaborate with therapists, case managers, and community partners to support client goals.
  • Document all contacts, services, and progress notes accurately and timely in required systems.
  • Monitor safety and risk, using basic crisis de-escalation and reporting concerns per policy.
  • Maintain professional boundaries, confidentiality, and compliance with ethical and legal standards.
  • Participate in team meetings, supervision/consultation, and required trainings as a contractor.
  • Manage schedule and travel to homes and community locations across assigned service areas.
  • Communicate effectively with families from diverse backgrounds, demonstrating cultural sensitivity.
