Full Time Regional Nurse Consultant
A Regional Nurse Consultant for memory care and assisted living facilities oversees and manages clinical operations, regulatory compliance, and staff training across multiple communities. This position is responsible for driving clinical excellence, managing state survey readiness, auditing documentation, and providing mentorship to Health Services Directors to ensure high‑quality, person‑centered care.
Essential Duties andJob Responsibilities
- Conduct internal regulatory/compliance audits, surveys, and inspections to ensure adherence to state, federal, and company requirements and track performance via key metrics.
- Monitor compliance with Oregon Administrative Rules (OAR), federal regulations, and company policies; prepare communities for annual state surveys and implement plans of correction.
- Implement quality improvement functions, policies, procedures, and processes that provide structure to the organization’s systems and facilities for regulatory and clinical standards compliance.
- Provide training, education, monitoring, and assistance to maintain the highest level of care and drive quality outcomes.
- Perform regular on‑site audits of clinical documentation, medication management, infection control, and resident care records.
- Offer ongoing training and mentoring to community‑based nurses (Health Services Directors) and nursing staff on clinical best practices, assessment tools, and care planning.
- Ensure all corporate procedures and systems are in place to promote the highest level of care across all facilities.
- Develop and adhere to a proactive facility visit schedule based on facility risk and clinical goals.
- Educate and assist regional clinical leaders to be survey ready at all times, ensuring documentation is complete and consistent with legal requirements.
- Review regional surveys for plans of correction and assess weekly reports for each facility to identify areas of concern or priorities.
- Provide reports and trends to company leadership and assist in developing improvement plans for various focus areas.
- Conduct compliance programs including mock surveys, system reviews, and routine facility visits to ensure ongoing compliance with federal/state and company policies.
- Facilitate regional meetings and orientation for newly hired Directors of Health Services.
- Be available 24/7 via telephone or on‑site as needed; return phone calls and email in a timely manner unless on vacation or sick.
- Support resident care needs by managing assessments, developing care plans, and facilitating care transitions.
- Conduct delegation assessments in accordance with Oregon state nursing practice standards.
- Current, unencumbered, active license to practice as a Registered Nurse in the state of Oregon.
- Nursing degree from an accredited college or university.
- Minimum of 3 years of experience as a Director of Health Services in Assisted Living/Memory Care facilities; five (5) years in Long Term Care and Skilled Nursing Facility settings.
- Demonstrable experience developing, implementing, and monitoring systems that ensure clinical excellence and compliance with regulations, standards, and company policies.
- Expertise in Infection Control/Antimicrobial Stewardship.
- Knowledge of Quality Measures and governmental programs to drive quality within Centers.
- In‑depth understanding of clinical and regulatory compliance requirements (Federal and State – OARs) for Assisted Living and Memory Care facilities.
- Experience with principles of Quality Assurance and QAPI.
- Strong communication skills and the ability to convey clinical knowledge effectively.
- Knowledge of emergency and disaster processes.
- Valid driver’s license and dependable vehicle.
- Experience with Electronic Medical Records and electronic documentation.
- Ability to maintain confidentiality of all resident care information in accordance with HIPAA.
