Position: Housing Case Manager- HCMS

Our services include a family homeless shelter, children's program, and housing case management designed to empower families toward stability and long-term prosperity. Through trauma-informed care, education, and collaborative partnerships, we strive to ensure that every family has access to safe, inclusive, and stable housing, along with the necessary support for holistic wellbeing and empowerment.

Job Summary

The Housing Case Manager, reporting to the Housing Program Manager, provides comprehensive case management services to participants eligible for supportive housing funded through the Metro funded Supportive Housing Services in Washington County. This role is responsible for developing and implementing individualized service plans, connecting participants with essential resources, and supporting long-term housing stability for 20-25 families. The position is primarily field-based with frequent travel throughout Washington County.

Starting Salary: $29.25/hour (plus $1/hour differential if bilingual in Spanish and English).

  • This is a full-time, non-exempt, on-site role.

Primary Responsibilities Case Management
  • Provide intensive ongoing case management services to households experiencing homelessness or housing instability
  • Develop and implement individualized service plans addressing housing goals, income, employment, mental health/substance abuse issues, and other risk factors
  • Support households in navigating the housing process, including housing location, landlord advocacy, and move-in support
  • Conduct regular home visits and check-ins to monitor client progress and provide ongoing support
  • Utilize a harm reduction/housing first model when working with households
  • Maintain accurate and up-to-date case notes in HMIS within 24hours
Community Engagement and Advocacy
  • Develop close working relationships with landlords, local service providers, and collaborative agencies
  • Advocate for clients' needs and respond to urgent requests for assistance from households or landlords
  • Participate in meetings with collaborative agencies and other community meetings as directed
Program Development and Compliance
  • Complete all forms and documentation required for the HCMS program thoroughly and completely
  • Participate in program evaluation and improvement initiatives
  • Stay current on best practices in supportive housing and homeless services
Required

Skills and Abilities
  • Proficient understanding and application of Trauma-informed Care, with ability to support individuals experiencing homelessness effectively
  • Strong communication skills, capable of engaging with diverse individuals and fostering an inclusive work environment
  • Excellent organizational and project management abilities, adept at multitasking
  • Commitment to respecting diversity and promoting equity in all aspects of work
  • Basic proficiency in Microsoft Office and case management software
Preferred Qualifications
  • Significant experience in homelessness services or housing case management preferred
  • A combination of education (such as a Bachelor's degree in social work, human services, or a related field) and lived experience may be considered in lieu of work experience
  • Demonstrated success working with culturally diverse populations and communities facing systemic barriers
  • Proficiency in crisis management and de-escalation techniques
  • Experience using HMIS (Homeless Management Information System) software or the desire to learn quickly after hiring
  • Fluency in Spanish and English (preferred, with pay differential)
Certifications & Licenses
  • CPR and First Aid Certification (or ability to obtain within 30 days of employment)
Travel Requirements
  • Travel between client locations, GNC offices, the GNC Shelter, and community partner organizations
  • Conduct weekly client meetings in-person and virtually to perform eviction prevention
  • Conduct monthly home visits after clients attain a higher level of stability
  • Mileage for approved work-related travel will be reimbursed
