Occupational Therapist - Home Health
Location:
Concord, Tilton & Manchester, NH Territory Pay: $45.00–$52.00 per hour
Schedule:
Full-Time | Monday–Friday | 8:00 AM–5:00 PM Productivity: 5 patient visits per day

As a Home Health Occupational Therapist, you'll provide individualized, one-on-one occupational therapy services to patients in their homes throughout the Concord, Tilton, and Manchester areas. You'll evaluate patients, develop personalized treatment plans, and help individuals regain independence with daily activities while collaborating with physicians, nurses, and the interdisciplinary care team.
Responsibilities- Perform comprehensive occupational therapy evaluations
- Develop and implement individualized treatment plans
- Help patients improve independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
- Evaluate upper extremity strength, coordination, cognition, perception, and functional mobility
- Recommend adaptive equipment, assistive devices, and home modifications to improve safety and independence
- Educate patients and caregivers on therapeutic techniques, fall prevention, energy conservation, and home safety
- Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and other members of the care team
- Complete timely and accurate documentation using Kan Time EMR
- Participate in interdisciplinary case conferences and care planning
- Bachelor's, Master's, or Doctorate in Occupational Therapy from an accredited program
- Current Occupational Therapist license in New Hampshire
- Strong communication and interpersonal skills
- Excellent organizational and time management abilities
- Comfortable using electronic medical records (EMR)
- Home health experience is preferred but not required
- We welcome Occupational Therapists from acute care, inpatient rehabilitation, outpatient therapy, skilled nursing, long-term care, or other clinical settings who are interested in transitioning into home health
