LICENSED PRACTICAL NURSES I-II (Job )
State of New Hampshire Job Posting – New Hampshire Veterans Home – 139 Winter Street, Tilton, NH 03276
Salary: $31.11 – $51.02 hourly
Direct‑Care & Nursing Stipend: up to $25.00/week
Hourly Differentials: $4.00 for 2nd shift, $7.00 for 3rd shift, $2.00 for weekends
Schedule: All shifts available; must work equivalent of one weekend per month
OverviewPart‑time Licensed Practical Nurses (LPN I and LPN II) are needed to provide quality nursing care to veterans in the New Hampshire Veterans Home.
LicensingMinimum license or temporary permit status required for both LPN I and LPN II. LPN II also requires certification by the National Federation of Licensed Practical Nurses (NFLPN), Inc. or recognized alternative.
Qualifications- LPN I –
Education:
Completion of a recognized practical nursing program approved by the New Hampshire Board of Nursing. - LPN I –
Experience:
No experience required beyond required training. - LPN I – License/Certification: Current practical nurse license or temporary permit pending exam.
- LPN II –
Education:
Same as LPN I. - LPN II –
Experience:
Three years as a Licensed Practical Nurse. - LPN II – License/Certification: Current practical nurse license and NFLPN certification.
- Completion of LPN II experience and certification requirements.
- Successful performance evaluation.
- Supervisor recommendation.
- Document nursing care and changes in resident records; record prescribed treatments.
- Administer medications, recording before/after notes to prevent medication errors.
- Use selected nursing process elements to contribute data for Plan‑of‑Care writing and updates.
- Supervise LNA staff; perform rounds to ensure care quality, cleanliness, and safety.
- Report routine and unusual facts to nurse manager and facilitate shift‑to‑shift hand‑offs.
- Consult with physicians and allied health professionals during in‑house visits.
- Provide nursing assistant support (direct ADL care, bathing, transfers, lifting).
- Educate residents on care plans and actions toward agreed goals.
- Brief family/guardians on care, progress, and sudden changes.
- Admit, transfer, and discharge residents as directed by attending physician.
- Maintain CPR competency and comply with departmental policies.
- Document nursing care and Plan‑of‑Care updates with detailed records.
- Administer medications and document effects, preventing polypharmacy issues.
- Contribute to Plan‑of‑Care development and revisions.
- Supervise LPN I’s and LNAs, performing rounds to ensure quality of care.
- Facilitate routine and unusual information hand‑offs between shifts.
- Consult with physicians and other health professionals during visits.
- Counsel residents on care plan goals per regulatory requirements.
- Brief family/guardians on care progress and sudden changes.
- Admit, transfer, and discharge residents per physician direction.
- Provide nursing assistant support (direct care, bathing, transfers, feeding, lifting).
- Collaborate with health providers to meet resident needs and communicate with intradepartment team.
- Maintain CPR competency and comply with policies and procedures.
