Job SummaryThe Licensed Practical Nurse (LPN) provides skilled nursing services to home health patients in accordance with physician orders, the individualized plan of care, and Medicare Conditions of Participation. Services are provided under the supervision of a Registered Nurse and in compliance with all federal, state, and agency regulations. The LPN promotes patient safety, quality outcomes, and continuity of care in the home setting.
Essential Duties and Responsibilities- Provides skilled nursing care in the patient’s home per physician-ordered plan of care and Medicare guidelines.
- Performs and documents skilled nursing assessments, vital signs, and patient observations within the LPN scope of practice.
- Administers medications and treatments as ordered by the physician and delegated by the supervising Registered Nurse.
- Provides wound assessment and dressing changes, catheter and ostomy care, diabetic care, and monitoring of patient status.
- Observes, assesses, and promptly reports changes in patient condition to the supervising Registered Nurse.
- Participates in care coordination and communicates with physicians, Registered Nurses, therapists, and other members of the interdisciplinary team.
- Provides patient and caregiver education related to diagnoses, medications, infection prevention, and safety.
- Completes accurate and timely clinical documentation in the electronic medical record in compliance with CMS, IDPH, and agency standards.
- Follows infection control, HIPAA, and patient rights requirements, and participates in case conferences, in-services, and quality improvement activities as required.
The LPN functions under the direction and supervision of a Registered Nurse in accordance with Medicare regulations, state nurse practice acts, and agency policy. Ongoing supervision and evaluation are provided per CMS and agency requirements.
Qualifications- Current, unrestricted Licensed Practical Nurse license in the state of practice.
- Minimum of one year of nursing experience, with home health experience preferred.
- Current CPR certification.
- Knowledge of Medicare home health regulations and documentation standards preferred.
- Ability to work independently in a home setting while following established clinical protocols.
- Reliable transportation and a valid driver’s license.
The position requires the ability to lift, transfer, and reposition patients as needed, up to 50 pounds. The LPN must be able to stand, bend, walk, and travel between patient homes in varying environmental conditions.
CompensationThis is a per-visit position paid at a rate of $55 per visit. Estimated annual compensation ranges from approximately $57,000 to $114,000, depending on visit volume, patient census, and availability.
