Responsibilities
  • Promote Elara Caring’s philosophy, mission, and administrative policies to ensure quality of care.
  • Evaluate patients to determine rehabilitation needs and develop a plan of care.
  • Admit the patient and assess speech‑language therapy and other needs per OASIS & PPS guidelines. Make initial visit, re‑evaluate, and conduct follow‑up assessments.
  • Initiate patient safety plan using available resources.
  • Communicate significant findings, problems, and changes to the appropriate supervisor, physician, and team members.
  • Participate in implementation and development of the Plan of Care, ensuring quality, continuity, and proper discharge planning.
  • Verify the Plan of Care before each visit and provide care per physician orders and established standards.
  • Write physician orders for additional visits and changes in the plan as needed.
  • Incorporate patient care goals into therapy care, evidencing documentation in therapy notes.
  • Perform direct speech‑language pathology services per accepted standards and as certified by the physician.
  • Select diagnostic and therapeutic materials appropriate to patient needs. Provide patients/caregivers with home therapy program instructions and modifications.
  • Ensure clinical notes reflect ongoing communication and coordination with physician and staff.
  • Maintain accurate and timely clinical, supply, and visit logs per office policy.
  • Work in a physically demanding, high‑stress environment.
  • Handle, lift, and transfer patients requiring full range of body motion.
  • Be available for irregular hours, including call hours if applicable.
Requirements
  • Master’s degree in Speech‑Language Pathology.
  • Current, unrestricted SLP license in the state of work.
  • Minimum one year of SLP experience in a clinical care setting.
  • Willing and able to travel within the branch/office coverage area.
  • Dependable vehicle with valid driver’s license and current auto insurance.
  • Must be willing to travel 50% of the time.
  • Must be able to sit, stand, bend, lift, and move intermittently and lift 50–100 lbs.
Compensation

The compensation range is $80 – $90 per point, with additional earnings for work beyond quota, bonuses, on‑call, or other performance‑based compensation. Compensation varies by education, experience, and geographic market.

Reporting Line

Reports to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
