Elder Life Management, now part of the BAYADA family, is seeking a compassionate, adaptable, and clinically strong Registered Nurse (RN) Care Manager to support older adults and their families in Tinton Falls, NJ with clients primarily in central jersey.

This is a unique opportunity for an RN who wants to use their clinical judgment in a more independent, relationship-driven role. As an RN Care Manager
, you will work directly with older adults and their families to assess needs, develop individualized plans, coordinate services, advocate across settings, and help clients remain as safe, supported, and independent as possible.

This role is ideal for a nurse who enjoys meaningful one-on-one relationships, problem-solving, home visits, and guiding families through complex and changing situations. It is not a shift-based bedside role, and it is not a traditional office-based clinical manager role. It is a community-based nursing role that requires flexibility, maturity, sound judgment, and the ability to pivot as client needs change.

  • Coordinate home care services and other service systems to support safety, well‑being, and quality of life
  • Conduct home visits and support clients in their own homes or other selected environments
  • Serve as an advocate for clients and families as they navigate aging, changing health needs, and care decisions
  • Review care management plans regularly and adjust services as client needs evolve
  • Support client autonomy while guiding appropriate service utilization

  • Build trust quickly with older adults and their families
  • Handle schedule changes and shifting priorities with professionalism and calm
  • Work independently while staying connected to the larger team
