Home Health RN - Part Time

Role Overview:

The Registered Nurse (Home Health) provides personalized, in-home nursing care to promote patient recovery, safety, and independence.

• Serve as the primary clinical partner for patients and families, delivering individualized care.
• Conduct comprehensive in-home assessments to develop patient-centered care plans.
• Perform skilled interventions such as wound care, infusions, medication management, and post-operative care.
• Educate and empower patients and caregivers for effective symptom and medication management.
• Monitor patient progress, identify changes early, and communicate with care teams to adjust plans.
• Coordinate across providers to ensure continuity and high-quality outcomes.
• Complete accurate documentation supporting compliance and quality metrics.

• Active RN license in the practice state.
• At least one year of clinical nursing experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Valid driver’s license, reliable transportation, and CPR certification or ability to obtain it.
