Location: Titusville and surrounding areas in Brevard County

Schedule: Flexible, based on patient needs and availability

Matrix Home Care is dedicated to helping patients remain safe, independent, and supported in their homes through compassionate, dependable, and high-quality care. We are seeking an experienced Registered Nurse to complete Start of Care assessments and skilled nursing visits throughout Titusville and the surrounding Brevard County area.

This is an excellent opportunity for a dependable RN who is comfortable working independently, managing patient care in the home setting, and coordinating with physicians, clinical staff, patients, and families.

• Complete Start of Care assessments and initial patient evaluations
• Perform skilled nursing visits, reassessments, recertifications, and discharge visits
• Develop and update individualized plans of care
• Monitor patient conditions and report changes to the appropriate clinical team
• Provide wound care, medication management, patient education, and disease-management support
• Coordinate care with physicians, therapists, caregivers, and other healthcare professionals
• Complete accurate and timely documentation in the electronic medical record
• Follow OASIS requirements and home health documentation standards
• Educate patients and families regarding medications, treatments, and care plans
• Maintain compliance with agency policies, state regulations, and professional nursing standards

• Active Registered Nurse license in Florida or an active compact nursing license
• Current CPR certification
• Previous home health experience preferred
• Experience completing Start of Care visits and OASIS documentation strongly preferred
• Strong clinical assessment and documentation skills
• Ability to work independently in the field
• Reliable transportation and willingness to travel throughout Titusville and surrounding areas
• Ability to communicate professionally with patients, families, physicians, and clinical staff
• Current driver’s license and auto insurance
