Responsibilities
- Provide care to clients according to the Plan of Care under the supervision of a Registered Nurse ("RN") in accordance with state and federal regulations and SHCS philosophy, policies and procedures.
- Administer medications and injections, ensuring adherence to the five rights of medication to reduce errors.
- Measure intake and output, perform tube feedings, monitor catheters, and record vital signs.
- Collect blood samples, obtain specimens for analysis, and perform other diagnostic and therapeutic procedures as required.
- Assist with activities of daily living and provide wound care, dressing as needed.
- Observe and report changes in client’s condition, conveying pertinent information to the RN.
- Teach family or responsible party about client care and promote client/family/caregiver education using verbal and written techniques that respect cultural, ethnic, and personal preferences.
- Supervise CNAs, ensuring progress and adherence to the Plan of Care and client satisfaction with services.
- Perform all aspects of client care in an environment that optimizes safety and reduces the likelihood of medical/health care errors.
- Complete and submit necessary documentation in accordance with state and federal rules, as well as company policies and procedures.
- Maintain currency of professional knowledge by participating in continuing education and attending required in‑services and training sessions.
- Participate in interdisciplinary team conferences and case conferences in accordance with company policy.
- Identify and assess appropriate resources to meet client and/or family needs to facilitate optimal client outcomes.
- Ensure the availability of equipment and supplies to support care plans, using equipment/supplies per plan of care and documenting per agency policy.
- Assist in billing audits and maintain confidentiality of company and patient information.
- Current State of Georgia License as a LPN.
- Minimum of one (1) year of nursing experience preferred.
- Home health experience is a plus.
- Current CPR/First Aid certification.
- Good organizational and communication skills.
- Copy of SSN.
- Valid driver's license/State .
- Negative PPB TB skin test within the last 12 months.
- TRAIN THE TRAINER CERTIFICATE.
- Background check and random drug testing required.
- Maintain working knowledge of home health coverage guidelines, admission criteria, documentation requirements, coding guidelines, and care planning.
- Conduct initial home visits, introduce services, confirm eligibility, and obtain required consents.
- Assess patient and caregiver willingness, ability, and barriers to learn patient care techniques; document responses.
- Outline aide care plan, oversee home health aide activities, and evaluate aide care every 14 days or per state payer requirement.
- Supervise CAN participation in patient’s plan of care and performance of skilled interventions per state regulations.
- Initiate the plan of care, conduct goal‑oriented visits, and ensure other nursing team members have necessary information.
- Provide patient/family teaching per Plan of Care; assess and document response to teaching.
- Advocate for the patient as required.
- Complete accurate, initial comprehensive head‑to‑toe assessment, OASIS, and other assessments to determine home care needs.
- Use assessment data to determine patient needs and establish primary and secondary diagnoses.
- Develop care plan, including skilled interventions, medical supplies/equipment, and ancillary services; write POC orders accordingly.
- Regularly evaluate patient progress, revise Plan of Care, and perform ongoing OASIS assessments.
- Identify discharge planning needs, implement community referrals, and determine patient readiness for discharge.
- Prepare clinical notes and required documentation within required time frames.
- Obtain and communicate physician orders to the appropriate team members.
- Track all assigned cases, schedule to meet patients’ needs.
- Meet agency productivity requirements and request PTO in advance per agency protocol.
- Coordinate with Clinical Supervisor regarding plan of care, overflow, weekend, and after‑hours nurse assignment.
