Zepf Center has been serving the Lucas County community for over 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care.

Zepf Center is a trauma-informed agency and environment for both patients and staff.

We are currently seeking a Full-Time Behavioral Health Assessment Clinician
. According to State, Agency and Program policies, guidelines and professional requirements, this position provides direct services to assigned consumer with major mental illness and a substance abuse diagnosis, specifically within the Criminal Justice system. These services are provided to assist in resolving or minimizing the effects of a consumer’s impairment through a wide range of skills, including assessment, monitoring, crisis intervention, education, linking and training in community resources, assistance in daily living skills, and obtaining necessary services for meeting basis human needs and entitlements.

This position participates in a treatment team and maintains professional record keeping.

It is expected and essential for all employees to meet their regular work schedule which may include rotation of shifts. Excessive absenteeism and/or tardiness will not be tolerated. All employees within this department will be required to learn all aspects of their position even when tasks/responsibilities are divided among staff member and/or shifts.

Essential Duties and Responsibilities:
  • Conduct comprehensive biopsychosocial assessments
  • Complete diagnostic evaluations and determine DSM-5 diagnoses
  • Determine appropriate level of care and placement recommendations using the ASAM
  • Perform risk assessments and administer standardized screening tools as required
  • Develop and document clinical impressions, initial treatment recommendations, and referral plans
  • Coordinate referrals to internal and external sources
  • Conduct regular substance use and mental health individual sessions as medically indicated per individual
  • Develop ISP’s with each individual client upon admittance to the program
  • Update client’s ISP’s every 90 days at minimum or as LOC indicated
  • Attend staffing and report patient progress.
  • Complete Discharge Summaries within time standards
  • Document all correspondence with BOP, parole officers, probation officers
  • Operate program activities within OhioMHAS, contractual, and CARF program standards and guidelines.
  • Maintain active communication with all agency treatment components to ensure patients’ smooth transition throughout the continuum of care.
  • Complete all other tasks as assigned.

DIRECT SERVICE:

Provides direct Criminal Justice services to an assigned caseload of consumers in accordance with the State mandates and established Agency and program policies, procedures, philosophy and consumer Individualized Service Plan.

  • Demonstrates the knowledge and skills to provide appropriate care to consumers with a substance abuse diagnosis including understanding their clinical needs, symptoms, and behaviors as well as the principles of growth and development over the life span. Is able to assess data about each consumer’s status and interpret the information needed to identify each individual’s requirements in relation to his or her age-specific needs and provide care that is appropriate.
  • Develops and coordinates treatment on all assigned cases.
  • Provides individual and group substance abuse and mental health counseling, trauma counseling, education, crisis intervention, urinalysis screenings, and SUD case management as appropriate per licensure.
  • Assess for psychoactive substance toxicity, intoxication and withdrawal symptoms.
  • Links for appropriate level of care or services per OHMAS standard of practice.
