The Hospice Clinical Supervisor leads the hospice clinical team to utilize a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required to meet hospice patient/family physical, emotional, psychosocial, and spiritual needs while coordinating available resources to promote quality and cost-effective outcomes through ongoing audits of medical records and clinical documentation to continually improve data quality in accordance with company standards and federal, state, and local standards, guidelines, and regulations
Essential Activities and Tasks Operations Management and Administration - 55%- Facilitates clinical team with the coordination and oversight of all direct and indirect patient/family care services ensuring accurate evaluation of patient/family physical, emotional, psychosocial and spiritual needs, and development and implementation of comprehensive care plans that address those needs by following current hospice nursing professional standards of practice and agency approved protocols. Supervises the implementation of the physician ordered hospice plan of care, by coordinating, auditing, overseeing, integrating, and modifying the IDG resources necessary to reach desired outcomes.
- Oversees Hospice IDG review, revision, and documentation of the individualized plan of care as frequently as the patient’s condition requires, but no less frequently than every 15 days.
- Organizes and oversees hospice IDG (interdisciplinary group) meetings which are held no less frequently than every
15 days. - Interprets operational indicators to detect census changes and increases/decreases in volume that could drive staffing levels, revenues, and/or expenses.
- Evaluates and approves the clinical documentation and approves electronic medical record workflow according to established standards.
- Reviews diagnosis coding and plan of care orders for all hospice patients in accordance with Medicare regulations.
- Assures all equipment, systems, and monitoring are in place to meet or exceed quality, customer service, utilization, and compliance standards in all company hospice programs.
- Collaborates with marketing team to manage processes, protocols, and timeliness of all referrals to meet the comprehensive needs of the patients.
- Manages the local component of the integrated electronic medical record and assures that the office and clinical staff are well trained and use the system productively and accurately according to company policies.
- Evaluates the effectiveness of the plan of care for hospice care delivery and collaborates with hospice clinical supervisors and hospice staff on best practice and desired outcomes.
- Evaluates workflow processes, administrative processes, and departmental processes.
- Monitors and maintains hospice revenue cycles and collection cycles and ensures accuracy in documentation and coding.
- Conducts supervised patient visits.
- May be asked to case manage to meet business needs.
- Participates in on-call rotation, as needed.
- May assume the role of Alternate Administrator during the temporary absence and/or vacancy of the Administrator.
