Job SummaryThe Manager, Community Health Initiatives leads the design, implementation, and continuous improvement of initiatives that integrate clinical care and community health to improve care, access, and health in the communities we serve.
The manager partners with clinical service line and hospital teams, philanthropy and grants, marketing-communications, and other stakeholders across the organization. The position also builds relationships and works closely with community organizations and stakeholders.
Required QualificationsEducation: Bachelor’s degree in healthcare administration, public health, social work, a related field, or an equivalent combination of experiences.

- Works collaboratively with diverse stakeholders to solve problems, resolve conflicts, and make effective decisions.
- Builds interpersonal trust and credibility with diverse stakeholders to develop implement effective initiatives and processes.
- Communicates effectively by developing and delivering multi-mode communications to different audiences.
- Strong ability to plan and coordinate complex projects, manage timelines, and lead teams to achieve project results.
- Collects, evaluates, and understands data to measure and share outcomes.
- Secures and manages grants and funding from public and private sources.
- Provide leadership, mentorship, and professional development opportunities to cultivate a culture of collaboration, innovation, and accountability.
Years of
Experience:
7-10 years of experience in community and public health, health access and improvement, or community program management, with at least 3 years of people leadership experience.
Education: Master's Degree in relevant field
License: BSN
