We are seeking a Social Worker for Ashanti Hospice to assist in the growth of our Hospice ministry.
Responsibilities- Conduct initial comprehensive psychosocial and bereavement risk assessments within five (5) calendar days after a patient’s election of hospice care.
- Facilitate and coordinate referrals to community resources, including financial assistance and agency services.
- Assist families in placement decisions for basic care, respite, or inpatient facilities as needed.
- Provide psychosocial and anticipatory grief counseling to patients, families, and caregivers.
- Assess psychosocial status related to coping capacity, and communicate findings to the interdisciplinary team.
- Participate in care planning, hold case conferences, and prepare social histories, evaluations, and interventions.
- Maintain and submit clinical documentation per agency guidelines, assist with discharge planning.
- Attend interdisciplinary team meetings to coordinate care plans and address social issues.
- Ensure respectful, dignified care and report complaints or potential abuse following corporate compliance policies.
- Mentor new personnel through orientation, field training, and in‑service education.
- Educate patients, families, and caregivers on disease progression and self‑care techniques.
- Strong communication, documentation, interpersonal, and computer skills.
- Ability to assess patient needs and develop individualized care plans.
- Ability to work independently in field settings and make sound judgments.
- Education: Master’s degree in social work from an accredited school of social work.
- Licensure: Valid Ohio driver’s license, reliable transportation, and active Ohio Social Work license.
- Experience: Minimum of 1 year in a health‑care field working with patients and families coping with terminal illness; hospice or home‑care experience preferred.
