Harbor is seeking a CME Hospital Liaison to join the team! This position supports the coordination of care for youth involved with the Child and Family & Community Mental Health Enhancement (CME) program through hospital partnership development, care coordination, service connection, and admission/discharge collaboration to help ensure timely access to services and continuity of care.

Harbor is seeking a CME Hospital Liaison to join the team! This position supports the coordination of care for youth involved with the Child and Family & Community Mental Health Enhancement (CME) program through hospital partnership development, care coordination, service connection, and admission/discharge collaboration to help ensure timely access to services and continuity of care.

Position is full-time and works remote and in the community.

Requirements Education
  • Bachelor’s degree in a mental health or related field.
Experience
  • At least 2 years of experience in behavioral health, providing services to youth and their families or caregivers, reporting and data management, and compliance.
  • Experience in at least one of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
  • Culturally competent or responsive with training and experience to manage complex cases.
  • Experience and qualifications to work with children and families who are experiencing SED, trauma, co-occurring behavioral health disorders and who are engaged with one or more child-service systems.
Licensure And Certifications
  • State of Ohio Clinical licensure; LSW, LPC or above.
  • Must maintain active licensure in good standing.
  • CANS certification required within 90 days of employment.
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
  • High Fidelity Wraparound trainings by COE, skill and competency-based trainings completed as necessary.
  • Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier.
Preferred Qualifications
  • Prior Wraparound or Intensive Home-Based Treatment experience.
  • Advanced degrees, certifications, and experience.
Responsibilities Essential Functions and

Primary Responsibilities:
  • Routine visits to hospitals to strengthen relationship and engagement.
  • Outreach IP referrals to connect to care coordination within 24 hours of notification.
  • Coordinate services for clients discharging from hospitals or acute settings.
  • Complete Safety Plan, prior to discharge, and review within 7 days of returning to community.
  • Ensure safe and timely transitions to community-based care prior to discharge.
  • Communicate with hospital discharge planners and providers for youth who are active in Care Coordination with county Care Coordinators.
  • Schedule follow-up services and ensure continuity of care for up to 30 days post discharge or until connected with ongoing Care Coordinator.
  • Monitor Clinisync for IP admission and discharge information.
  • Monitor the High Acuity Report, IP Report, and other applicable reports from Aetna.
  • Attend High Acuity Staffing Weekly.
  • Provide ongoing care coordination services for ongoing cases if needed.
  • Conduct care coordination assessments and documentation per CME Manual and Ohio Revised Code/Ohio Administrative Code.
  • Monitor client engagement and service access post-discharge.
  • Address barriers to care including transportation, housing, and services.
  • Maintain compliance with documentation and regulatory standards.
