In the rich Franciscan tradition of compassionate, quality care, Franciscan Care Center is a residential and rehabilitation facility for the care of the elderly and those in need of extensive rehabilitation services. We are seeking a Licensed Social Worker (LSW) to join our team! This position requires an active LSW (Licensed Social Worker) license in the state of Ohio.

• Help assess resident needs and develop individualized care plans.
• Provide emotional support and guidance to residents and families.
• Coordinate with the interdisciplinary care team to ensure holistic care.
• Assist with admissions, discharges, and transitions of care.
• Maintain accurate, confidential documentation and resident records.
• Communicate with outside agencies to connect residents with resources.
• Participate in care plan meetings and departmental discussions.
• Support the overall well-being of our residents through presence and compassion.
• In addition to the essential job specific duties listed above, the Licensed Social Worker (LSW) shall be required to perform all duties (essential and non-essential) in a manner consistent with the mission and values (compassion, excellence, inclusion, integrity, collaboration) of CHI Living Communities and will be evaluated on such basis. Furthermore, every employee must abide by all campus, departmental, and safety policies, rules, and regulations.

• Bachelor’s Degree in Social Work from an accredited college or university.
• Licensed Social Worker in the state of employment.
• Previous experience with mature adults preferred.
