Visiting Angels Toledo is looking for a self-motivated, empathetic, and proactive LPN or RN to join our team as Client Care Coordinator! We provide non-medical homecare to seniors and veterans in our community - Lucas County and surrounding areas and Bowling Green. Role Overview:

As Client Care Coordinator at Visiting Angels, you will be responsible for overseeing client care and maintaining relationships with clients and their families. In this role, you ll collaborate closely with our caregiving team to ensure high-quality non-medical care for seniors and veterans. Working on-site in Toledo, OH, you’ll have the opportunity to support our mission of providing Liberty and Dignity through Service.

Key Responsibilities:

You will coordinate care services for clients, ensuring their needs are met by creating and maintaining individualized Care Plans.

You will assist in the recruitment, training, and supervision of caregivers.

Using a company car, you will travel to clients homes to conduct assessments and supervisory visits, updating Care Plans as needed.

You will maintain effective communication with clients, families, and staff to address concerns and feedback.

Using a company cell phone, you will assist in an on-call rotation to ensure 24/7 accessibility to our clients and caregivers.

You will ensure compliance with all regulatory and company standards related to client care.

Qualifications:

Previous experience in a supervisory or coordination role within healthcare or homecare.

Strong communication and interpersonal skills.

Ability to develop care plans based on client assessments.

Proficiency in using technology for scheduling and client management.

Compassionate approach to service and care for clients.

Bonus If You:

Have experience with senior care or familiarity with the specific needs of the elderly.

Possess certifications related to care coordination or patient management.

Are bilingual, with proficiency in languages beyond English.

This position is Full-Time, M-F, 8am-4pm on-site in Toledo, OH and is open to applicants from Michigan and Ohio. Some afternoons/weekends may be required.
