Concept Rehab is seeking a Certified Occupational Therapy Assistant (COTA) to join our post-acute rehabilitation team in a Skilled Nursing Facility in Toledo, Ohio. Job Responsibilities Under the supervision of a Licensed Occupational Therapist, the Certified Occupational Therapy Assistant (COTA) will:
- Deliver occupational therapy treatments according to physician orders and established plans of care
- Assist patients in improving performance with activities of daily living (ADLs)
- Support rehabilitation goals related to mobility, balance, strength, range of motion, cognition, safety awareness, and functional independence
- Monitor and report patient progress, response to treatment, and changes in condition
- Collaborate with Occupational Therapists, nursing staff, physicians, families, and interdisciplinary team members
- Participate in care conferences, discharge planning, and treatment planning meetings
- Complete accurate and timely documentation, treatment notes, and billing records
- Educate patients, caregivers, and facility staff regarding therapeutic techniques, safety strategies, and restorative programs
- Current Certified Occupational Therapy Assistant (COTA) license in Ohio
- Graduate of an accredited Occupational Therapy Assistant program
- Strong communication and interpersonal skills
- Commitment to compassionate, patient-centered care
- Experience in Skilled Nursing, Long-Term Care, Rehabilitation, or Post-Acute Care preferred but not required
- New graduates are encouraged to apply
