We are seeking a dedicated and compassionate Wound Care Nurse to join our team at White Oak Rehabilitation and Nursing at. As a Wound Care Nurse, you will play a crucial role in providing high-quality care to our patients with complex wound cases. You will work closely with our interdisciplinary team to develop and implement individualized plans of care, monitor patient progress, and educate patients and their families on wound care and management.

• Develop and implement individualized plans of care for patients with complex wound cases
• Monitor patient progress, identify areas of improvement, and adjust care plans accordingly
• Provide wound care treatments, including debridement, dressing changes, and other therapies needed
• Educate patients and their families on wound care and management, including proper wound cleansing, dressing changes, and medication administration
• Collaborate with the interdisciplinary team, including physicians, physical therapists, occupational therapists, and speech‑language pathologists, to ensure comprehensive care
• Document patient information, medications, treatments, and progress in a timely and accurate manner
• Participate in quality improvement initiatives to ensure high-quality patient outcomes

• Current licensure as a Registered Nurse in the State of Ohio
• Bachelor's degree in Nursing or related field
• Minimum of 1 year of experience as a Wound Care Nurse (new graduates welcome to apply!)
• Excellent communication and interpersonal skills
• Ability to work effectively as part of an interdisciplinary team
