Job Summary: As a Hospice Registered Nurse (RN) Case Manager at Ohioans Home Healthcare & Hospice, you will play a key role in an organization with a reputation for unsurpassed quality care, a close‑knit, family‑friendly culture, and a supportive management team as we begin to introduce hospice services to our Toledo, OH territory.

Responsibilities:

  • Assess, plan, and implement patient care
  • Educate patients and families on comfort care processes, procedures, and medications
  • Address various patient needs
  • Maintain continuity of care and build one‑on‑one relationships with patients and families
  • Monitor medication compliance, IV therapy, injections, and provide education on administering injections
  • Ensure compliance with special diets, weight monitoring, and utilization of intravenous nutrition such as TPN and tube feeding
  • Provide instruction on the use of inhalers, aerosol machines, oxygen, and equipment care
  • Deliver post‑operative care, continuous monitoring of chest pain, blood pressure, edema, and education on signs and symptoms of further cardiac problems
  • Conduct dressing changes, wound vac therapy, assess wound healing, and identify signs of infection
  • Inform patients about disease processes of various chronic conditions
  • Provide pediatric care, including PICC care, tracheostomy care, chemotherapy, parenteral nutrition, feeding, medication administration, and education to parents/caregivers

Qualifications:

  • One year of comfort care experience
  • CPR certification
  • Ability to travel for in‑home patient visits
  • Exceptional time‑management skills
  • Excellent interpersonal and customer service skills
  • Availability for an on‑call rotation
