Position OverviewThe Private Duty Nurse (PDN) is based in Toledo, OH (Zip: 43614) and will care for a client 24 hours per week. The position is part‑time with the option to grow to full‑time and add additional clients in surrounding areas.
Immediate Client Needs- The client is an adult male requiring bi‑pap vent care, medication administration, seizure monitoring, vitals, and ventilator education and assessments.
- Daytime hours, 24 hours per week, with flexibility in exact schedule.
- At least one year of direct patient vent‑care experience is required.
- The candidate must be able to push, pull, and roll the client and meet all basic job physical requirements.
- Immediate start with 24 hours per week.
- No on‑call, weekend, or overnight travel required unless desired.
- Team‑centered culture with quality and compliance support, training, and referral bonuses.
- Mileage reimbursement for travel between multiple clients when applicable.
- LPN: $28–$30 per hour
- RN: $35–$40 per hour
- Graduate of an accredited nursing school.
- Active LPN or RN license in Ohio.
- Minimum one year of home‑health industry experience and one year of nursing experience.
- Preferred:
Additional home‑health experience and CPR/First Aid certificates. - Driver’s license, reliable transportation, and willingness to undergo a background check.
- Excellent attendance record with past employers.
- Strong interpersonal, verbal, and documentation skills.
- Ability to meet all basic job physical requirements.
Performs skilled nursing functions under the direction of a Registered Nurse Case Manager.
Responsibilities include:
- Providing professional nursing care, medication monitoring, and client education.
- Communicating status changes to the RN and physician as required.
- Documenting care promptly in the clinical record.
- Adhering to safety, infection control, and agency policies.
- Assisting with patient transport, positioning, and equipment use.
- Participating in quality improvement activities.
- Frequent standing, walking, bending, and occasional sitting.
- Push/pull force: 10–20 lbs; occasional 20–30 lbs. Lift: 5–10 lbs; infrequent 10–30 lbs.
