Hospice Registered Nurse RN - Case Manager

Otterbein is a not‑for‑profit, faith‑based ministry and national leader in retirement living communities and long‑term care. We are seeking an experienced Hospice RN Case Manager to assist in the growth of our Hospice ministry. The role focuses on providing compassionate care to patients with life‑limiting illnesses and supporting their families and caregivers.

Full‑time, Monday–Friday 8:00 a.m.–5:00 p.m. with on‑call rotation as scheduled.

• Manage a primary caseload of hospice patients.
• Provide skilled professional nursing care, including assessment, care planning, and symptom management, in compliance with Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws, and agency policies.
• Collaborate with interdisciplinary team members—nurses, chaplain, social worker—to develop and implement individualized care plans.
• Educate patients, families, and caregivers on end‑of‑life care options and support services.

• Education:
  
  Bachelor’s degree in nursing preferred; graduate of an approved nursing program.
• Licensure/Certification:
  Valid Ohio Registered Nurse (RN) license.
• Experience:
  
  Minimum 2–4 years of direct patient care in hospice; case‑management experience preferred.
• Experience with patients diagnosed with end‑of‑life conditions preferred.
