Licensed Practical Nurse (2600045Y)

Position:
Licensed Practical Nurse at Northwest Ohio Psych Hosp. Compensation: $26.76 per hour.

Location:

930 South Detroit Avenue, Toledo, Ohio 43614.

Schedule:

Full‑time, 3:00 pm – 11:30 pm. Union: OCSEA. Pay range: #10 on the OCSEAPay Range Schedule.

Responsibilities
  • Provide medication, education, and empowerment to individuals.
  • Observe, evaluate, and record patient behavior, condition, and response to medication.
  • Monitor patient health including vital signs, treatment plan progress, and activity participation.
  • Serve as a member of a multi‑disciplinary team, creating a safe and positive environment through safety and observation rounds and group facilitation.
Qualifications
  • License to practice nursing as a licensed practical nurse issued by the Ohio Board of Nursing pursuant to Section 4723 Ohio Revised Code and proof of completion of an approved course in medication administration.
  • Licensed Practical Nurses who administer dialysis treatment must possess Certification in IV Therapy pursuant to Section 4723.17 Ohio Revised Code.
  • Official transcripts for all post‑secondary education, coursework, or degrees listed on the application. Must be submitted within five (5) business days of request.
  • Additional qualifications will be evaluated against experience, education, and disciplinary record.

Background Check and Drug Testing

The final candidate selected for this position will be required to undergo a criminal background check. Criminal convictions do not necessarily preclude an applicant from consideration for a position. An individual assessment of an applicant’s prior criminal convictions will be made before excluding an applicant from consideration. Final selected applicants will also be required to submit to a urinalysis to test for illegal drug use prior to appointments.
