LTSS Service Coordinator (Case Manager)
Location Requirement: Hiring statewide across Ohio. Must reside in the state of Ohio.
The MyCare Ohio health plan is to deliver high‑quality, trauma informed, culturally competent, person‑centered coordination for all members that addresses physical health, behavioral health, long‑term services and supports, and psychosocial needs.
Responsibilities- Conduct face‑to‑face program assessments using standardized tools and motivational interviewing techniques to evaluate individuals’ waiver (LTSS/IDD), behavioral health or physical health needs.
- Use identification tools to detect members with potential clinical health care needs, coordinate their cases with the clinical management and interdisciplinary team, and provide care coordination support.
- Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, or disabilities, ensuring cost‑effective utilization of long‑term services and supports.
- Document members’ short‑ and long‑term service goals in collaboration with their chosen care team, including caregivers, family, natural supports, service providers, and physicians.
- Identify members who may benefit from alternative levels of service or other waiver programs.
- Mentor and pre‑train new staff and participate in process improvement initiatives.
- Submit utilization/authorization requests to utilization management with documentation aligning with the individual’s care plan.
- Report critical incidents to internal and external parties such as state and county agencies (Adult Protective Services, Law Enforcement).
- Assist in and participate in appeals, member grievances, fair hearings, and state audits.
- BA/BS degree and a minimum of 2 years of experience working with a social work agency, or an equivalent combination of education and experience.
- Strong preference for case management experience with older adults or individuals with disabilities.
- BA/BS in Health or Nursing preferred.
Salary for this position ranges from $26.19 per hour to $32.75 per hour for candidates working in the Columbus, OH location.
