POSITION INFORMATION

Job Title: Transitional Care Coordinator

Department: Value Based Care

Reports to: Manager, Care Management and Coordination

POSITION SUMMARY

The Transitional Care Coordinator (TCC) will be responsible for the planning, organizing, implementing, and coordinating activities contributing to the nursing process to provide a team-based, patient-centered approach. This role will work collaboratively with physicians and the care team to promote health services that are appropriate, safe, and efficient. Strong organizational and positive relationship-building skills are essential for this position.

ESSENTIAL JOB FUNCTIONS AND ACCOUNTABILITIES

  • Supports the patient’s transition from Emergency Department (ED) discharge to primary care services or ambulatory specialists as appropriate.
  • Contributes to a focused health, self-management, socio-economic, behavioral, and risk status assessment with patients. Supports treatment adherence.
  • Communicates with agencies and community service providers, identifies available community and health resources, and helps the patient and family access needed care and services.
  • Maintains a professional, collaborative, and productive relationship with providers and the care team.
  • Participates in quality improvement projects as needed and implements quality improvement initiatives.
  • Promotes effective collaboration amongst care team members, including the patient and family. Identifies and builds upon the assets and strengths of all members of the care team.
  • Effectively utilizes all tools available to support care coordination. Maintains a high level of proficiency with tools within EMRs and other applications intended to support patient care.
  • Works with a team of health care professionals and will require the flexibility to shift responsibilities as necessary.
  • Participates in program, department, and clinic team meetings as indicated.
  • Other duties as assigned.

REQUIRED QUALIFICATIONS

  • Education
    :
    Graduate of an accredited LPN program
  • License and/or Certification:
    • Current unencumbered Ohio Practical Nursing Licensure
    • BLS Certification
  • Skills
    :
    • Excellent interpersonal, written, and verbal communication skills
    • Demonstrated ability to work collaboratively.
    • Ability to meet deadlines, prioritize, and organize time.
  • Years of Experience
    :2+ years of applicable experience

PREFERRED QUALIFICATIONS

  • Education
    : RN or RN BSN preferred
  • License and/or Certification:
  • Current unencumbered Ohio Registered Nursing Licensure
  • Skills
    :
    • Care coordination experience preferred.
    • Experience with quality improvement processes preferred.

    Experience:

    5+ years of applicable experience

WORKING CONDITIONS

  • Works in a professional office/ambulatory clinic environment with possible moderate to high stress levels.
  • Work requires occasional travel within and to UTP and UTMC locations, and UT campuses. Primarily inside well-lighted, well-ventilated areas.

GENERAL REQUIREMENTS

  • Works in a safety-conscious manner that supports and ensures safe work practices.
  • Complies with all UTP and other applicable UT policies and procedures and local, state, and federal regulations.
  • Interacts in a tactful, diplomatic, and humanistic manner with patients, visitors, and co-workers to provide a patient-centric environment.
