Otterbein Senior Life is seeking an experienced MDS Nurse Transition Leader to guide MDS processes and care planning across Ohio/Indiana locations. You will mentor staff, manage PDPM/casemix tasks, and collaborate on QA initiatives within a supportive, resident-centered culture.
The role requires a BSN and at least 5 years of MDS experience in long-term care, with strong EMR/PCC familiarity and English communication skills. Travel up to 100% may be needed based on transition leadership needs.
