Mercy Health St. Vincent Medical Center in Toledo, OH seeks a nurse case manager to oversee patient transitions of care from hospital to post-acute settings.
You will ensure payer approvals for services and maintain documentation supporting patient status, level of care, and DRG. The role requires an Ohio RN license (BSN preferred) with a minimum of 3 years clinical experience;
Case Management certification is preferred and should be obtained within 3 years of hire.
