Licensed Practical Nurse (LPN) – Home Health
Part-Time / Flexible Schedule $25 per Skilled Nursing Visit + Mileage Reimbursement

We are seeking a dependable part‑time LPN to provide skilled nursing visits in patients' homes.   • $25.00 per visit (approximately 40-45 minutes) and $37.50 per visit on designated holidays

  • Flexible scheduling based on patient needs and nurse availability

  • Paid travel time between patients at $12.50 per hour, plus mileage reimbursement when using a personal vehicle (Arista nurse vehicles include an assigned gas card)

  • Paid training and staff meetings; paid travel time between patients

  • 24/7 on‑call clinical manager support

Responsibilities of the Licensed Practical Nurse (LPN):
  • Provide skilled nursing services under the direction of a registered nurse and in accordance with authorized orders and the established plan of care

  • Observe and document the patient's condition; implement ordered nursing interventions; and contribute to the planning, implementation, and evaluation of care

  • Administer medications and treatments and provide other ordered skilled care within the LPN's scope, training, and demonstrated competency

  • Provide or reinforce patient and family education and communicate promptly with the RN, physician, and other members of the care team

  • Complete accurate, timely documentation and follow agency emergency procedures

Qualifications for the Licensed Practical Nurse (LPN):
  • Graduate of a state‑approved practical nursing program

  • Current, valid Ohio LPN license in good standing

  • Available for part‑time, per‑visit work, including rotating weekends with other LPNs

  • Current CPR certification; valid driver's license; current auto insurance; and reliable access to a vehicle

  • Successful completion of required pre‑employment screening, including BCI background check, BMV check, Ohio Board of Nursing license verification, and drug screen

  • Basic computer skills and ability to complete electronic clinical documentation
