Role:
A highly skilled and compassionate Registered Nurse (RN) is needed to provide patient-centered home health care, including in-home visits, telehealth consultations, and administrative duties.
Responsibilities:Conduct comprehensive assessments, develop care plans, perform clinical interventions (medication administration, wound care, point-of-care testing, phlebotomy, IV therapy), document patient interactions accurately, educate patients and caregivers, support care transitions, and collaborate with interdisciplinary teams.
Key Skills &Qualifications:
Active RN licensure, minimum 3 years of clinical experience, CPR certification, proficiency with Electronic Health Records and telehealth platforms, strong communication and organizational skills, ability to work independently, and experience with diverse populations. Prior home care, hospice, palliative, or geriatrics experience is preferred.
Additional Requirements:Valid driver’s license, reliable auto insurance, ability to travel locally, lift up to 20-50 lbs, and perform in various work conditions including standing for long periods and driving to patient locations.
Key Duties:Deliver urgent in-home care, perform safety evaluations, support care transitions, contribute to interdisciplinary team meetings, and maintain compliance with regulatory standards. The role involves flexible hours and local travel, with a focus on delivering high-quality, patient-first care.
