LTSS Service Coordinator (Case Manager)
Hiring statewide across Ohio. Must reside in the state of Ohio.
LocationThis field‑based role primarily operates in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. Alternate locations may be considered if candidates reside within a commuting distance from an office.
Responsibilities- Manage service coordination for a designated caseload in specialized programs.
- Collaborate with individuals to lead the Person Centered Planning process, documenting preferences, needs, and goals.
- Conduct assessments and create comprehensive Person Centered Support Plans (PCSP) and backup plans.
- Participate in interdisciplinary care rounds with Medical Directors to establish integrated care plans.
- Engage the individual's support network and oversee the management of their physical health, behavioral health, and long‑term services and supports in compliance with state and federal regulations.
- Perform face‑to‑face program assessments using defined tools and motivational interviewing techniques to evaluate program participation and coordinate services such as LTSS/IDD, BH, or PH needs.
- Identify members with potential clinical health care needs, coordinate care with the interdisciplinary team, and serve as the single point of contact.
- Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, or disabilities to ensure cost‑effective utilization of services.
- Document short and long‑term service goals with the member’s care team, including caregivers, family, and providers.
- Make recommendations for alternative levels of service or other waiver programs.
- Mentor new staff, assist in training, and support process improvement initiatives.
- Submit utilization/authorization requests to utilization management with appropriate documentation.
- Report critical incidents to relevant internal and external agencies (e.g., Adult Protective Services, Law Enforcement).
- Participate in appeals, hearings, grievances, and state audits.
- BA/BS degree and a minimum of 2 years working in a social work agency, or an equivalent combination of education and experience.
- Strong preference for case management experience with older adults or individuals with disabilities.
- BA/BS in Health or Nursing preferred.
