Service Coordinator
Full Time | Day Shift
Pay: $41,600-$45,760 annually
based on experience
Serving multiple Genacross communities:
Luther Ridge, Luther Hills, Luther Grove, Covenant Harbor I, Covenant Harbor II, and Oak Harbor

- serving the needs of our community with exceptional care, innovation, and support.
- Assess resident needs and coordinate appropriate supportive services and resources
- Maintain accurate resident records, assessments, progress notes, and documentation
- Conduct annual resident apartment visits and update resident files as needed
- Monitor resident concerns and communicate important issues to leadership promptly
- Assist residents in navigating available community programs and benefits
- Report suspected abuse, neglect, or exploitation in accordance with regulatory requirements
- Complete monthly reports and maintain compliance with program requirements
- Support partnerships with local organizations, faith communities, and service providers
Assist with PASSPORT Certification documentation and reporting - Serve as a Transition Coordinator for assigned Home Choice Program referrals
- Participate in required training and professional development opportunities
- Help ensure high-quality, resident-centered service delivery
- Valid driver's license
- Bachelor's degree in social work or related field preferred
- LSW/LISW preferred
- Strong oral and written communication skills
- Understands directions, communicates and responds to inquiries promptly
- Excellent human relation skills
