Zepf Center has been serving the Lucas County community for 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care.

Position Summary

We are currently seeking a full‑time Youth Community Based Case Manager. The purpose is to provide a wide range of community support and coordination of services to people experiencing serious mental illness and/or addiction challenges to ensure their needs are addressed. The Case Manager supports clients in the development of short‑ and long‑term goals, acquires community resources using a person‑centered approach, and coordinates crisis intervention services to support emotional and physical health and wellness.

    Monday - Friday; 8:30am - 4:30pm.
Essential Duties And Responsibilities
  • Assess client strengths and needs, conduct care coordination, and develop treatment plans. Monitor and evaluate client progress toward goal attainment and modify plans as needed.
  • Maintain contact with clients as indicated, provide support to achieve goals, and offer active outreach, including crisis intervention and management.
  • Link and refer clients to other services, programs, and agencies as needed and assist with securing entitlements.
  • Advocate for clients while ensuring compliance with evidence‐based practices and cost‑efficiency.
  • Collaborate with supervisors and team members regarding client care and interagency coordination.
  • Provide face‑to‑face contact with hospitalized clients to facilitate discharge planning and coordination of services.
  • Assist clients in tracking appointments and developing independence skills.
  • Provide backup support for other staff and complete required documentation in the electronic health record within 48 hours of interaction.
  • Follow agency policies, participate in community work, staff development, agency projects, committees, meetings, and other related duties as requested.
  • Complete required training including CPR, CPI, new hire, Sanctuary, and annual boosters, plus any additional trainings assigned.
  • Drive safely in all‑weather conditions to support client needs.

  • Incorporate Continuous Quality Improvement (CQI) techniques into daily work activities.
  • Participate in CQI process teams and/or support CQI initiatives.
Working Conditions

The position is dynamic and involves extensive interaction with individuals and families, cooperation with multiple community organizations, and computer‑based data entry and monitoring. It demands cultural and racial diversity awareness and sensitivity. Employees may interact with a diverse group of individuals who may display anger or frustration, and some may be under the influence of drugs or alcohol. The employee will be exposed to emotionally charged issues and must respond compassionately in a trauma‑informed manner.

Requirements
  • HS Diploma and/or equivalent with three years of experience OR Bachelor’s or Master’s degree in a related field.
  • Valid driver’s license. Those who drive personal vehicles during business must provide proof of insurance.
  • QHMS certification required throughout employment.
  • Social Work license and mental health experience preferred.
