ESSENTIAL FUNCTIONS:

This position provides comprehensive and supportive services to Pathway, Inc. clients. The incumbent is responsible for maintaining accurate records and supporting documentation for each client, while meeting outcome-based performance targets established by the Director of Community Resources.

  • Provides and delivers comprehensive and intensive case management services directly to program participants.
  • Identifies community resources and develops community partnerships to expand available services and eliminate participant barriers.
  • Assists clients by verifying income to determine eligibility for supportive services and enters client information into the electronic systems databases.
  • Maintains accurate and organized client records for potential audits.
  • Monitors the progress of program participants receiving supportive services and provides follow-up for clients exiting the program.
  • Conduct face-to-face interviews with clients to determine eligibility for services.
  • Collaborates with staff on outcome reviews, team meetings, client inquiries and other job related matters.
  • Participates in community outreach efforts to promote program awareness and engagement.
  • Performs other related duties as assigned.

QUALIFICATIONS:

  • Experience in social work processes, practices and client support services.
  • Knowledge of community resources and related services that support individual needs.
  • Familiarity with teaching aids and techniques that promote independent living skills.
  • Experience providing client services, coaching and training support.
  • Demonstrates the ability to work effectively with individuals facing barriers to employment or self-sufficiency.
  • Proficiency in the computer applications, internet navigation, and data entry systems.
  • Strong interpersonal and communication skills, with the ability to engage complex clients courteously and empathetically.
  • Ability to communicate clearly and professionally with diverse groups and represent the agency positively.
  • Strong organizational, multitasking, and record-keeping skills.
  • Ability to work independently with minimal supervision.
  • Positive, team-oriented attitude with a commitment to collaboration and service excellence.
  • Ability to build and maintain relationships with social service agencies faith-based organizations, employment services, and other community partners.
  • Willingness and ability to travel for outreach and training purposes.
  • Must possess a valid driver’s license, access to a reliable vehicle, and provide proof of liability insurance.
EDUCATION:
  • A high school diploma/GED is required.
  • An associate degree in Social Work is preferred.
  • Candidates with some postsecondary education in Social Work, Psychology, or related field, along with four (4) years of experience providing and/or coordinating supportive services to help individuals achieve their goals, will also be considered.

WORKING CONDITIONS:

  • The position operates in a High-volume client service office serving low-income individuals in crisis, including managing calls for Agency staff and programs. Requires the ability to operate telephone, personal computer, fax machine and copier. Some extended office hours may be required.
  • ADA Considerations: Duties are primarily performed in an office setting with normal office noise. Local and occasional long-distance travel is required, which may involve driving in normal or hazardous weather conditions. Occasionally standing, sitting, or walking for extended periods is expected. The position may require lifting up to thirty (30) pounds.
