Description

The mission of the Tunnel to Towers Foundation is to honor the sacrifice of firefighter Stephen Siller who laid down his life to save others on September 11, 2001. We also honor our military and first responders who continue to make the supreme sacrifice of life and limb for our country. The Tunnel to Towers Foundation is committed to eradicating homelessness amongst our veterans.

The Housing Stability Coordinator serves as a consistent, proactive point of contact for formerly homeless Veterans housed through the Homeless Veteran Program. Through a structured, long-term outreach schedule, the Coordinator monitors housing stability, strengthens Veteran engagement, and identifies emerging risks employing early intervention before challenges escalation into a return to homelessness.

This is a high-impact, relationship-driven role focused on sustaining housing success across each Veteran's first two years of tenancy.

Requirements
  • Execute a structured schedule of follow-up calls to Veterans at defined milestones throughout their first two years of housing.
  • Confirm current housing status and verify that rent and utility obligations are being maintained.
  • Conduct wellness checks to identify emerging needs, such as food insecurity, mental health concerns, or employment challenges.
  • Serve as an early-warning system: when a Veteran is at risk of eviction or struggling with the transition, coordinate timely intervention with the case management team.
  • Document every interaction, housing status update, and identified need in the program's CRM (Client Relationship Management) software to support long-term outcome tracking.
  • Provide warm hand-offs to VA programs or VA-contracted and grantee providers for specialized services identified during outreach.
Qualifications
  • High school diploma required;
    Associate's preferred.
  • Three to five years of experience in Veteran services, outreach, case management, or social work.
  • Working knowledge of the Housing First model, the VA HUD-VASH program, and CRM (Client Relationship Management) software.
  • Strong interpersonal, organizational, and documentation skills.
  • Bilingual (English / [additional language]) strongly preferred.
  • Pre-employment screening, including a background check, is required.
Working Conditions
