Impact Health is seeking a compassionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our clinical team serving patients within skilled nursing care facilities in Tolland, CT.

Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, dedicated to prioritizing patient care. We specialize in Medical, Psychiatric, Wound Care and Palliative Care.

We offer a collaborative environment that values expertise and independence with a flexible Monday through Friday schedule and no nights, weekends, or holidays.

Compensation and Benefits
  • Earning potential:
    From $110,000 up to $200,000 (salary based on experience + monthly bonuses)

Key Responsibilities
  • Assess, develop and communicate comprehensive treatment plans using evidence-based best practices
  • Documentation within 24-48 hours along with placing relevant orders for diagnostics and medications
  • Monitor and manage acute and chronic conditions
  • Compliance with facility, state and federal regulations/protocols
  • Support quality initiatives and process improvement plans
  • Collaborate with interdisciplinary team, nurse managers, physicians and specialists
  • Educate and consult patients and families regarding course of treatment
Qualifications
  • Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • Minimum 2 years of post-certification experience
  • Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
  • Proficient with EHRs and clinical technology
  • Strong communicator with a collaborative mindset
